About Us

Axle Health builds AI scheduling and workforce management software to empower in-home healthcare providers to deliver exceptional, personalized care right where patients feel most comfortable—at home.

Some of the biggest providers in the country use our software (like Cityblock Health and U Penn Health System) to improve the utilization of their field teams by 17%+, and automate the operational burden of scheduling and dispatching.

Our platform includes proprietary logistics algorithms to optimize scheduling, a mobile app for field staff, an operations dashboard for office teams, engagement and booking tools for patients, and a wide range of integrations and external APIs for modular access.

We’re a team of top-tier engineers and operators, backed by $14m from top investors such as Y Combinator, Pear VC, and F-Prime, applying our intellect and logistics experience to upend how home healthcare is delivered.

About You - The Role

Axle Health is hiring a Senior Software Engineer to join our Interoperability team, the group that connects our AI-powered scheduling and workforce management platform to the systems home health organizations already run. Most of those are EHRs and EMRs that weren't built to integrate cleanly: inconsistent APIs, thin documentation, and vendors you have to connect with for answers. Getting data in and out of them reliably is what makes Axle Health part of a clinician's day, and this role owns that work end to end.

The person who thrives here wants to own the hard problem. When a customer reports that something looks wrong, you dig in, figure out where the issue lives, put together a plan, and see it through while keeping the customer and our delivery team informed. You'll scope your own projects, make the technical calls, and drive them from the first conversation through production support without someone mapping out each next step. This is a hands-on individual contributor role on a small team, and the integrations you build carry real weight for our customers and our product.

You've done this before. You've built and maintained integrations with external or legacy systems you didn't control, from design through the production issues that come after launch. You've worked somewhere operations depend on data arriving on time, whether that's healthcare, logistics, field services, or another operations-heavy business, and you've done it at a startup or growth-stage company.

What You'll Do
  • Integration Design and Build: Design and build integrations between Axle Health's platform and EHR and EMR systems, including older systems with inconsistent APIs and limited documentation.

  • End-to-End Ownership: Scope integration projects, make the technical decisions, and carry them from the first conversation through launch and ongoing production support.

  • Customer Issue Resolution: Investigate customer-reported issues, find the root cause across our systems and theirs, and execute a plan to resolve them while keeping the customer and account team informed.

  • Vendor and Partner Coordination: Work directly with EHR vendors' and partners' technical teams to close documentation gaps and get integrations working as intended.

  • Project and Timeline Management: Sequence your integration work, set realistic milestones, and keep Engineering, Professional Services, and Product Management updated on progress and risk.

  • Reliable, High-Volume Systems: Build backend services that handle data for hundreds of thousands of patients, with close attention to data integrity, security, and uptime.

  • Monitoring and Resilience: Build monitoring and failure handling at the edges where external systems are least predictable, so issues surface before customers notice them.

  • Code Quality and Documentation: Hold a high bar in code reviews and write clear documentation so integration knowledge is shared across the team.

Requirements

Must Haves

  • 7+ years of professional software engineering experience - including end-to-end ownership of integrations with external or legacy systems, from build through production support

  • Track record of owning projects with significant autonomy - scoping the work, making the technical calls, and driving to completion without step-by-step direction

  • Comfortable investigating customer-reported issues directly and explaining the problem, the plan, and progress to technical and non-technical stakeholders

  • Demonstrated ability to architect scalable, performant, and secure systems

  • Experience at a fast-paced startup or growth-stage company

  • Bachelor's degree

Nice to Haves

  • Backend expertise in Node.js and TypeScript (1-2+ years) and hands-on experience designing and working with SQL databases - PostgreSQL preferred

  • Experience with EHRs or EMRs and healthcare interoperability standards (FHIR, HL7)

  • Integration experience in logistics, field services, or another operations-heavy domain - EDI, ERP, WMS, POS, or similar systems

  • Background in a domain where data integrity and security are non-negotiable

  • Experience building monitoring and alerting for systems with significant data volume or uptime requirements

  • Working knowledge of CI/CD, DevOps practices, and build systems

  • Familiarity with the Axle Health tech stack: ExpressJS, GraphQL, React, React Native, PostgreSQL, AWS, Datadog

Tools You May Use

  • ExpressJS + TypeScript

  • PostgreSQL

  • AWS

  • Datadog

  • Git / GitHub / Graphite

  • Django + Python3

  • GraphQL + React + React Native

  • Claude Code and Cursor

What you need to know about the Los Angeles Tech Scene

Los Angeles is a global leader in entertainment, so it’s no surprise that many of the biggest players in streaming, digital media and game development call the city home. But the city boasts plenty of non-entertainment innovation as well, with tech companies spanning verticals like AI, fintech, e-commerce and biotech. With major universities like Caltech, UCLA, USC and the nearby UC Irvine, the city has a steady supply of top-flight tech and engineering talent — not counting the graduates flocking to Los Angeles from across the world to enjoy its beaches, culture and year-round temperate climate.

Key Facts About Los Angeles Tech

  • Number of Tech Workers: 375,800; 5.5% of overall workforce (2024 CompTIA survey)
  • Major Tech Employers: Snap, Netflix, SpaceX, Disney, Google
  • Key Industries: Artificial intelligence, adtech, media, software, game development
  • Funding Landscape: $11.6 billion in venture capital funding in 2024 (Pitchbook)
  • Notable Investors: Strong Ventures, Fifth Wall, Upfront Ventures, Mucker Capital, Kittyhawk Ventures
  • Research Centers and Universities: California Institute of Technology, UCLA, University of Southern California, UC Irvine, Pepperdine, California Institute for Immunology and Immunotherapy, Center for Quantum Science and Engineering
