Provide hands-on home health nursing including medication management, wound care, and health monitoring. Use AI-supported documentation tools to streamline clinical notes, focus on patient interaction, and maintain required certifications and licensure.

Serve as the hospital-based RN liaison to coordinate and facilitate home health and hospice admissions and transitions. Assess referrals, educate hospital staff and families, document in EMR, coordinate discharge planning, arrange services and consents, and promote Value-Based care initiatives while building relationships with post-acute providers and internal teams.

Provide LPN/LVN hospice nursing under RN supervision: implement and document Plan of Care, observe and report patient data, manage caseloads, anticipate symptom progression, educate patients/families, perform triage and medication desk duties, collaborate with hospice team, and ensure regulatory and policy compliance.

Serve as the primary link between hospital teams, JV agencies, and community providers to evaluate referrals, coordinate home health and hospice admissions, educate staff and families, document care in EMRs, and support discharge planning and Value-Based Enterprise initiatives.

Coordinate and facilitate transitions from hospital to home health or hospice by evaluating referrals, educating staff and families, arranging admissions, documenting in EMRs, collaborating with discharge teams, and supporting GIP hospice program expansion while tracking outcomes and promoting post-acute relationships.

Review and validate Home Health OASIS assessments and key Plan of Care elements for CMS compliance and accurate outcome measures. Provide education and feedback to clinical staff, analyze OASIS accuracy trends, ensure timely processing to meet CMS billing and regulatory requirements, and support quality improvement initiatives.

Obtain and process insurance authorizations for ordered healthcare services, verify coverage, enter authorization data into systems, coordinate with MDs and agency staff, communicate with patients and care teams, and support team authorization efforts while maintaining confidentiality.

Lead configuration, testing, release management, and maintenance of Workday Integrations and Extend. Translate business requirements into Workday solutions, build reports and dashboards, drive process improvements, maintain documentation and training materials, handle tickets and feature release testing, and collaborate cross-functionally to optimize People systems and employee experience.

Partner with divisional leaders to deliver HR strategy, organizational design, leadership development, performance management, benefits administration, dispute resolution, and HR metrics. Drive HR programs, intake and triage employee issues, ensure policy compliance, coordinate projects (open enrollment, audits), and support culture and workforce planning across Western US operations.
