Provide hands-on home health nursing including medication management, wound care, and health monitoring. Use AI-supported documentation tools to streamline clinical notes, focus on patient interaction, and maintain required certifications and licensure.

Serve as the hospital-based RN liaison to coordinate and facilitate home health and hospice admissions and transitions. Assess referrals, educate hospital staff and families, document in EMR, coordinate discharge planning, arrange services and consents, and promote Value-Based care initiatives while building relationships with post-acute providers and internal teams.

Provide LPN/LVN hospice nursing under RN supervision: implement and document Plan of Care, observe and report patient data, manage caseloads, anticipate symptom progression, educate patients/families, perform triage and medication desk duties, collaborate with hospice team, and ensure regulatory and policy compliance.
