Join our team and make a positive impact on the lives of others! We are looking for a Strategic Partnership Program Manager for our National Expansion team.
This role is remote with required domestic travel to various U.S. locations for national expansion; up to 70% domestic travel.
Department & Position Overview:
The Strategic Partnerships Program Manager will play a pivotal role in expanding our relationships with key Medicaid stakeholders across multiple states, driving awareness of home and community-based services, and facilitating growth opportunities in future state markets.
This role will be a visible representative of FreedomCare and responsible for developing and maintaining strategic relationships with state government officials, case management agencies, area agencies on aging, managed care organizations (MCOs), and community health organizations. Partnering closely with the Government and Public Affairs team, the Strategic Partnership Program Manager will help to shape our external presence, strengthen community ties, and identify new opportunities that drive growth and help more people access the care and support they need.
Success in this role is measured by strong partnerships, increased visibility in key markets, and the organizational growth these relationships help drive.
Responsibilities will include:
Develop and Execute Outreach Strategies
- Design and implement outreach strategies to engage community-based organizations, referral partners and key Medicaid stakeholders.
- Identify and pursue opportunities for collaboration with community organizations, government agencies, businesses, and other strategic partners.
- Partner closely with Government and Public Affairs to ensure outreach efforts support broader advocacy and policy goals.
Stakeholder Engagement
- Build and maintain strong relationships with state officials, MCOs, case management agencies, and local health partners.
- Serve as the organization’s key point of contact for Medicaid-related partnerships and stakeholder engagement.
- Represent FreedomCare at public meetings, forums, and community events to build visibility and trust.
- Act as liaison to local, state, and federal entities in partnership with the Government and Public Affairs team.
Community & Partner Outreach
- Design and oversee community programs, events and initiatives that promote awareness of FreedomCare’s services.
- Host workshops, town halls, and information sessions to raise awareness about our programs and foster trust with the community.
- Collaborate with internal departments to integrate community engagement efforts across the organization and initiatives.
Strategic Growth & Development
- Identify partnership opportunities that support expansion in new states and increase access to services in both new and existing states.
- Turn stakeholder relationships into tangible opportunities for the organization’s growth and increased access to services.
- Partner with cross-functional teams to ensure alignment between partnership opportunities, operational capacity, and market strategy.
- Define and track KPIs to evaluate partnership effectiveness and regularly report insights to senior leadership to inform strategic decisions.
External Communications & Awareness
- Act as a primary point of contact for community inquiries and feedback.
- In partnership with Marketing and Government and Public Affairs, create and distribute outreach materials and digital content that share FreedomCare’s mission and programs.
- Support brand awareness by communicating the organization’s impact and services clearly and consistently.
- Ensure all public-facing communications align with compliance standards.
Key Success Metrics
- New Market Development: Number of partnerships that support our entry into new states.
- Stakeholder Engagement: Strength and growth of relationships with MCOs, state officials, and community partners.
- Referral Impact: Impact of outreach communications, outcomes from events and stakeholder meetings. Increase in referrals tied to partnership efforts.
- Cross-functional collaboration: Successfully aligning with internal teams to support key initiatives.
Ideal Candidate Will Possess:
- Bachelor’s degree; master's degree in a related field is a plus
- 5+ years of relevant experience in healthcare partnerships, community outreach, business development, public relations; experience in homecare preferred
- Proven experience in stakeholder engagement and success navigating complex healthcare and government landscape
- Strong track record in sales, with a demonstrated ability to acquire new clients and drive business growth
- Strong understanding of state Medicaid programs
- Requires extensive travel (up to 70%) to foster in-person connections and ensure effective engagement with stakeholders
- Excellent communication skills, including written, verbal, and interpersonal abilities, with the ability to effectively engage diverse audiences and drive interest through targeted messaging.
- Strong organizational skills and attention to detail, with the ability to manage multiple projects and priorities simultaneously
- Critical thinking and problem-solving abilities, with a creative approach to developing innovative outreach strategies and initiatives
- Strong cross-functional collaboration skills, specifically with Government and Public Affairs, Marketing and Compliance teams
- Deep understanding of community dynamics and public sector engagement
- Passion for community engagement, social impact, and making a difference in the lives of others
- Proactive and self-motivated with a passion for improving healthcare delivery and patient outcomes

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $115,000 and $135,000 per year at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
