Trellis Chino
Shift: 11:00 PM - 7:00 AM (NOC)
Schedule: 4 Days On / 2 Days Off Rotation
Pay Range: $29.00 - $33.00 per hour, DOE

Trellis Chino is a 59-bed short-term skilled nursing facility conveniently located near Chino Valley Medical Center. Our facility features private suites and a patient-centered approach focused on providing exceptional clinical care and outstanding resident experiences.

We are currently seeking compassionate and dedicated Licensed Vocational Nurses (LVNs) to join our NOC shift team. Whether you are an experienced nurse or a recent graduate looking to begin your career in skilled nursing, we invite you to apply.

Essential Duties and Responsibilities
  • Administer medications and treatments according to physician orders
  • Accurately document patient care using electronic medical records
  • Monitor and assess resident conditions, reporting changes as appropriate
  • Provide care for residents with acute and chronic healthcare needs
  • Supervise and support Certified Nursing Assistants (CNAs)
  • Communicate effectively with residents, families, physicians, and interdisciplinary team members
  • Utilize critical thinking skills to make sound clinical decisions
  • Ensure compliance with all facility policies, procedures, and regulatory requirements
Qualifications
  • Current California LVN license in good standing
  • Strong clinical assessment and documentation skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently and collaboratively within a team environment
  • Compassionate and resident-focused approach to care
