22.00 - 28.00 USD Per Hour

Working Location:

Community Based Fresno County

Working Hours/Shift:

Monday-Friday, 8am to 5pm. Working hours are approximate and may include flexibility as needed to meet client needs.

What you bring to SBHG:

Education & Experience: One of the following pathways is required:

  • An associate degree in a social service field plus two (2) years of experience in a mental/behavioral health setting.

    • Or

  • A bachelor’s degree in a social science field plus six (6) months of experience (preferred).

Note: Relevant internship experience may be substituted for a portion of the required work history

Licensure & Compliance: 

  • A valid California Driver’s License and eligibility to meet agency driving criteria.

How you will make a difference: 

Provides services to clients in the office, community, and their homes. Travels within the community and may transport clients using a personal vehicle. Works with clients and families to provide case management services and support their needs. Participates in treatment team meetings to help ensure client goals and recommendations are followed. Maintains accurate and timely documentation in the Electronic Medical Record system. Assists with client and family assessments to identify strengths and needs, monitors progress toward treatment goals, and communicates updates to the primary therapist.

Division/Program Overview:  

  • Population Served: Serving adults, 18 plus
  • Facility & Setting: Outpatient community-based services and support. Delivers client services throughout Fresno County within designated county limits. May be required to transport clients/client family members utilizing personal vehicle.
  • Clinical Services: Individual Counseling.
  • Skills & Services: Medication management, Housing, Education, and Vocational Support services.
  • Role specific Benefits & Reimbursements: Mileage reimbursement included at a rate of $0.85 per mile and Monthly stipend included (Cell phone / Internet)
