Job Purpose:
- The Case Manager I at Behavioral Health Services Inc is responsible for providing comprehensive case management services to clients with behavioral health needs. The role involves coordinating care, advocating for clients, and ensuring access to necessary resources and services to promote their overall well-being and recovery.

Key Responsibilities:
- Conduct thorough assessments of clients' needs, strengths, and challenges to develop individualized care plans.
- Coordinate and monitor the delivery of services, ensuring that clients receive appropriate interventions and support.
- Advocate for clients' needs and rights within the healthcare system and community resources.

Qualifications

Required Education:
- Bachelor's degree in Social Work, Psychology, Counseling, or a related field from an accredited institution.

Required Experience:
- Minimum of 1 year of experience in case management within a behavioral health setting.
- Experience working with diverse populations and understanding of cultural competence in service delivery.
- Prior experience in developing and implementing individualized care plans.

Required Skills and Abilities:
- Strong organizational skills with the ability to manage multiple cases efficiently.
- Excellent communication skills, both verbal and written, to effectively interact with clients, families, and healthcare professionals.
- Ability to conduct thorough assessments and develop comprehensive care plans.
- Proficiency in using case management software and maintaining accurate client records.
- Strong problem-solving skills and the ability to work independently as well as part of a multidisciplinary team.
