Position Summary:

Counsel and support young adults (THPP 18-21 and THP 18-24) in developing basic like skills, identifying community resources, and planning for their own self-sufficiency.  

Essential Duties:

· Acquires, maintains, teaches and demonstrates knowledge of Continuum of Care, Shared Core Practice Model, evidence-based practices, and trauma-informed care and maintains a professional environment that is consistent with Title 22, licensing regulations, and established agency policies.

· Act as an agent of the agency in the role of Legal Guardianship to Unaccompanied Refugee Minors including all responsibilities guardianship entails. 

· Documents the provision of services in case files including crucial legal documentation that can affect the legal status of the young adult.

· Manage logistics such as obtaining required documents, assembling records, entering records into electronic database including documentation of crucial legal and data elements utilized by the Federal and State government. 

· Act as the hub of coordination and communication with community partners to ensure all medical, mental health, education, recreation, and daily living needs are met.

· Provide 24 hour case Management services to children and youth. 

· Develop and ensure a safe and supportive environment for young adults.

· Assist young adults to develop Transitional Independent Living Plan (TILP) and other required plans/case management and record keeping for the Foster Care Program. 

· Provide individual and group instruction to young adults in basic life skills.

· Provide individual and group instruction to young adults in job-readiness.

· Assist young adults in identifying and accessing resources in their home community.

· Assist young adults in obtaining documents such as birth certificates, ID cards, etc.

· Instruct and support youth’s access to public transportation.  

· Assist young adults in obtaining safe and appropriate housing.

· Assist and provide education to young adults about higher education opportunities.

· Regularly visit young adults living in community-based setting to encourage and support their transition to independence in accordance with their TILP.

· Meet with young adult for face to face case management services or schedule regular contact meetings as outlined per agency and Community Care Licensing standards.

· Present psychosocial needs, strengths and concerns of young adults during supervision, interdisciplinary meetings, and care coordination meetings.

· Report all serious incidents, allegations, grievances, and sensitive situations to Clinical Director immediately.

Minimum Qualifications:

BA/BS required and in related field preferred, as is a minimum of two years working directly with transitional age youth. Working with special needs populations such as pregnant/parenting young adults, young adults with mental illness or behavioral problems, system-involved youth, and/or immigrant/refugee populations strongly preferred.

Requirements

· Case Managers assigned to mono-lingual clients must be proficient in the youth’s primary language or must be able to work effectively through interpreters. Bilingual preferred.  

· Must possess and maintain a valid, unrestricted California driver’s license and provide safe, reliable transportation of youth to necessary appointments and activities. Will be expected to travel to the Moreno Valley FC office, as needed.

· May be required to obtain a Class B California Commercial Driver’s License (Class C with Endorsement) with a passenger’s endorsement within three months of hire.

· Must be able to successfully complete and maintain required certifications and training including, but not limited to First Aid/CPR.
