Teams at Wellnest Emotional Health & Wellness

Provide community-based case management, develop and monitor treatment plans, link families to resources, document services in EHR, attend team meetings and represent agency at community events. Respond to crises as part of Intensive Services Division.

Serve as primary liaison to elected officials, agencies, and community partners to advance policy, advocacy, and public affairs. Develop legislative relationships, monitor policy, coordinate events and site visits, lead advocacy campaigns, and integrate public affairs with communications to increase organizational visibility and support mental health, housing, and youth services.

The Case Manager provides support and services to promote children's emotional well-being, conducts assessments, leads groups, assists families with resources, and provides client rehabilitation services.
