Teams at Wellnest Emotional Health & Wellness

Provides community-based case management, behavioral interventions, advocacy, assessments, referrals, and rehabilitation services for children, youth, adults, and families. Develops and monitors treatment plans, coordinates with therapists and external agencies, supports caregivers with behavioral and family stressors, documents services in EXYM, and connects clients with resources. Work occurs in homes, schools, communities, and offices, including occasional evening or weekend hours.

Provides clinical oversight and field-based supervision for mental health programs. Supervises clinical providers, reviews treatment plans and case records, conducts quality audits, monitors service delivery and compliance, supports complex cases, evaluates staff performance, coordinates with public agencies, and develops training and procedures. Requires an active California clinical license, at least two years of post-licensure experience, supervision training, a valid California driver’s license, vehicle access, fingerprint clearance, and TB testing.

Provides intensive, field-based case management to transition-age youth experiencing homelessness. Develops service plans, coordinates housing, healthcare, mental health, substance use, education, employment, and benefits resources, and supports housing stability. Conducts assessments, outreach, crisis intervention, referrals, documentation, and care coordination while maintaining a caseload of approximately 20 participants. Requires collaboration with property management and community agencies, transportation to appointments, and occasional evening, weekend, and on-call coverage.
