Deliver hybrid in-field and remote enhanced care management: assess needs, develop care plans, coordinate providers and community resources, document in EMR/CRM, conduct outreach, and promote wellness and independence for vulnerable members.

Lead a regional sales team to grow enhanced care management enrollment by developing regional sales strategy, securing referral partnerships, managing pipelines and metrics, coaching field Community Liaisons, and collaborating with marketing and clinical teams. Frequent travel within Northern California to support team and cultivate partnerships.

Provide field and remote care management for vulnerable members: assess needs, develop and update care plans, coordinate with providers and social services, conduct outreach, document in EMR/CRM, arrange transportation, and report monthly to ensure program compliance.
