Teams at Team Select

Provide and oversee individualized patient-centered nursing care for home health patients, coordinate with physicians and multidisciplinary teams, complete mandated documentation (OASIS, PAR), perform assessments, medication reconciliation, patient/family education, supervise ancillary staff, participate in QA/QAPI, and be available for on-call coverage.
