Teams at Santa Barbara Neighborhood Clinics (SBNC)

Provides direct patient care, health education, care coordination, and chronic disease management for clinic patients. Responds to messages, coordinates follow-up after ED/hospital visits, manages medication refills per protocol, performs panel management using disease registries, and supports quality improvement and case management including occasional home visits.

Provide community outreach and education, connect patients to community services, assist with Medi-Cal and Covered California enrollment, facilitate health promotion classes and programs, represent SBNC at events, maintain program statistics, and support patient access to care.

Under supervision, performs medical billing tasks including scrubbing visits, posting charges, completing billing batches, answering patient billing inquiries, verifying insurance and eligibility, applying payments/adjustments, preparing claims, coordinating with clinical staff, and maintaining HIPAA-compliant billing records.
