Teams at Pair Team

Provide relationship-based outreach and ongoing case management for high-need Medicaid/Medicare members. Conduct telephonic/SMS engagement, develop health action/shared care plans, coordinate referrals and appointments, support clinical team with delegated tasks, meet encounter metrics, and perform occasional in-community visits (2-3x/month). Use CRM and standard office tools to document and collaborate.

Maintain caseload, conduct telephonic/SMS follow-ups, create Health Action/Shared Care Plans, coordinate appointments/referrals, support clinical team with delegated tasks, meet encounter metrics, and perform occasional (2-3x/month) community on-site visits.
