Provide hands-on supportive services in an affordable supportive housing community: assess resident needs, design and deliver programs, perform outreach, coordinate referrals, develop service plans and partnerships, track outcomes, supervise interns/volunteers, and represent the property in local collaboratives to promote housing and community stability.

Provide intensive case management to formerly homeless individuals with chronic medical and co-occurring disorders. Coordinate services with VA and housing partners, complete assessments and service plans, facilitate support groups, conduct crisis intervention, attend client appointments, maintain documentation and HMIS data, and support housing retention using harm reduction and trauma-informed practices.

The Case Manager coordinates services for formerly homeless individuals with chronic conditions, utilizing best practices and collaborating with various agencies to enhance housing retention and individual care management.
