Provide intensive case management to formerly homeless individuals with chronic medical and co-occurring disorders. Coordinate services with VA, housing authorities, property managers and community providers; conduct assessments, create and update treatment plans, facilitate groups, provide crisis intervention, attend client appointments, maintain documentation and HMIS records, and support housing retention. Work may include evenings, weekends, and travel up to 50%.
