Provides restorative nursing interventions under licensed nurse supervision in a long-term acute care hospital. Supports patients’ functional independence, safety, and physical, mental, and psychosocial well-being; reports condition changes; assists with quality reviews, safety activities, interdisciplinary care, administrative efficiency, and team meetings. Requires a high school diploma or GED and BLS certification. Rehab Tech or Restorative Nursing Aide experience is preferred.

Provides case management in an inpatient rehabilitation hospital, including pre-admission screening, patient assessments, individualized treatment planning, therapeutic interventions, care documentation, patient and family education, insurance pre-certification, and coordination of continued treatment. Maintains therapeutic relationships, supports functional independence, participates in professional development, and contributes to client relations and marketing activities.
