Manage end-to-end enrollment for prospective PACE participants: build relationships, process applications and documentation, conduct home/center/virtual visits, remove enrollment barriers, coordinate with outreach, clinical and interdisciplinary teams, maintain accurate records, and support outreach and performance tracking.

Lead nursing and clinic teams at a PACE center to ensure high‑quality, participant‑centered care. Oversee clinical competency, onboarding, training, regulatory compliance, care transitions, KPIs, and clinic operations. Support quality initiatives, maintain audit readiness (PACE/DHCS/CMS), and provide direct clinical care as needed.

Lead and manage all operational, clinical, financial, and administrative aspects of the Habitat Health PACE program. Oversee program development, staffing, budgets, regulatory readiness, QAPI, marketing, community relations, and provider/contracts. Supervise Medical Director and staff, ensure compliance with state and federal PACE requirements, support strategic planning, and represent the organization to partners and the community.
