Provides autonomous after-hours hospice nursing support during evenings, nights, and weekends. Responsibilities include visits and admissions, telephone triage, dispatching field staff, coordinating prescriptions and equipment, assisting patients and families, and documenting calls. The role requires a licensed Registered Nurse capable of handling stressful situations, supporting grieving families, traveling throughout the service area, and performing physical tasks.

Provides direct hospice nursing care, including assessments, symptom and pain management, medication evaluation, patient and family education, documentation, equipment preparation, and coordination with interdisciplinary care teams. The role requires maintaining the plan of care, using aseptic technique, participating in quality improvement, and supporting end-of-life patients and families. Candidates must hold an active state LVN/LPN license and have three years of nursing experience.
