Provide personal care and comfort to hospice patients under RN supervision, including bathing, feeding, vital signs, mobility assistance, light housekeeping, documentation, respite care, and reporting condition changes to the case manager.

Serve as a community-facing liaison to build and maintain relationships with referral sources, receive and coordinate hospice referrals, conduct community education and needs assessments, complete social work assessments and patient/family education, participate in IDT, bereavement, and QAPI activities, document care, and support in-service and continuing education programs.

Provide direct end-of-life nursing care, assess patients, implement and document plans of care, manage pain/symptoms, educate patients and families, coordinate hospice services, and participate in quality improvement and professional development.

Supervise clinical staff and ensure delivery of quality hospice home-care services. Review referrals, perform home visits and record reviews, assess eligibility, guide clinicians, assist hiring and orientation, support QAPI and education programs, and ensure policy compliance.

Coordinate hospice admissions and daily office operations: triage calls, initiate referrals, verify insurance and MD licensure, track census and OASIS transmission, support regulatory compliance, assist with orientation, and communicate status to the Executive Director.
