The AL Wellness Coordinator plays a vital leadership role in delivering exceptional, person-centered care within a high-end assisted living community designed for active older adults. Reporting to the Wellness Director, the Wellness Coordinator ensures the coordination, implementation, and ongoing evaluation of resident care services in full compliance with California Title 22 regulations while upholding the community’s elevated standards of hospitality, dignity, and quality of life.

 

The Wellness Coordinator serves as a primary clinical and operational liaison among residents, families, care staff, and interdisciplinary teams, ensuring individualized care plans are executed seamlessly and reflect residents’ preferences, independence, and wellness goals.

 

Schedule: Sunday - Thursday

Compensation: $38.00- $41.00 per hour 

 

Key Responsibilities

  • Coordinate and oversee resident care services in accordance with Title 22, Division 6, Chapter 8, ensuring compliance with all applicable licensing and regulatory requirements.
  • Assist in the development, implementation, and periodic review of Individualized Service Plans (ISPs) based on comprehensive assessments and resident needs.
  • Observe residents’ physical, cognitive, and psychosocial status and promptly report changes to the Wellness Director and appropriate licensed professionals.
  • Support residents in maintaining maximum independence while ensuring safety, dignity, and choice in daily living activities.
  • Ensure care services align with Title 22 requirements, including but not limited to:
    • Admission agreements and appraisals
    • Ongoing reassessments and service plan updates
    • Medication assistance oversight (as applicable)
    • Documentation standards and record maintenance
  • Participate in licensing inspections, audits, and surveys; assist with corrective action plans as directed.
  • Ensure unlicensed staff provide care within their permitted scope under Title 22 regulations.
  • Provide day-to-day guidance, coaching, and support to care staff to promote high performance and regulatory compliance.
  • Assist with onboarding, training, and mentoring of care team members, emphasizing resident-centered care and hospitality standards.
  • Observe staffing patterns to ensure adequate coverage and continuity of care.
  • Serve as a point of contact for residents and families regarding care services, questions, and concerns.
  • Participate in care conferences and communicate care plan updates with professionalism, empathy, and transparency.
  • Foster trust-based relationships that reflect the community’s commitment to personalized, luxury living experiences.
  • Collaborate closely with the Wellness Director, wellness nurses (if applicable), life enrichment, dining, and operations teams to support holistic resident wellness.
  • Support transitions of care, including move-ins, level-of-care changes, and coordination with external providers as appropriate.
  • Maintain accurate, timely, and confidential resident records in compliance with Title 22 and company standards.
  • Assist with quality improvement initiatives focused on resident satisfaction, care outcomes, and service excellence.
  • Other duties as assigned

 

Qualifications & Requirements

  • Current Licensed Vocational Nurse (LVN) certification required.
  • High school diploma or equivalent required; AA/AS degree preferred.
  • Minimum of 2–3 years of experience in assisted living, Residential Care Facilities for the Elderly (RCFE), or similar senior living environments.
  • Demonstrated working knowledge of California Title 22 regulations for assisted living/RCFE communities.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to balance regulatory compliance with a hospitality-driven, resident-first approach.
  • Experience in a high-end or luxury senior living environment preferred.
  • Proficiency with electronic health records (EHR) or senior living management systems preferred.
