Minimum Requirements:

  • Completion of a 3-year Family Medicine, Internal Medicine, or Emergency Medicine residency
  • Active ABMS or AOA Board Certification in Family Medicine or dual boarded Internal Medicine + Pediatrics
  • 3+ years post residency experience in Urgent Care or Primary Care outpatient setting
  • Active and unrestricted state medical license in state of residence; residing in an IMLC state is required
  • Multi-state licensure preferred 
  • Experience and ability to provide care for all ages across the lifespan (newborn to geriatric) 
  • Must maintain Medicare active status
