WELLNESS NURSE

Full Time: Sunday-Thursday

Sundays/Mondays: 12:30-9:00pm

Tues-Thursday: 11:00am-7:00pm

GENERAL SUMMARY:

The Wellness Nurse supervises the medication assistance program and is responsible for evaluating the physical, medical, mental and social conditions of the residents. This position shall respond to community emergencies as needed.

PRINCIPLE DUTIES:

  • Supervises the Medication Assistance Program to comply with RCFE regulations and ensuring that documentation and assisting with medication is accurately completed. Audits medication administration records on a regular basis.
  • Assists the Director and/or supervises the direct care of the residents being provided by the Medication Technicians in order to maintain quality resident care. Conducts regular in-services and training of staff on relevant topics. Maintains resident roster and special needs list to ensure it is current.
  • Serves as a customer service role model for Medication Technicians.
  • Responds to on-site emergencies with the emergency response team.
  • Responsible for assisting the Director in the completion of Resident evaluations and maintaining annual and significant change of condition evaluations current with appropriate care plan(s) in place.
  • Coordinates and conducts various health related clinics on campus.
  • Serves as knowledgeable resource for residents in navigating health resources.
  • Actively participates in Disaster, Safety and other campus committees.
  • Assists the Director in planning for campus wide health education for residents and staff.
  • Assists with resident and employee flu campaign.
  • Other duties as assigned by Department Director.

QUALIFICATIONS:

  • Current California License in good standing with Board of Nursing required. Preferred LVN.
  • Current First Aid and CPR/AED certification required.
  • A minimum of two years experience as a nurse.
  • A minimum of one year as a wellness nurse in assisted living or a CCRC with knowledge of Title 22 RCFE Regulations preferred.
  • One year experience as a home health nurse a plus.
  • Must possess strong clinical skills, and have strong customer service, leadership, and interpersonal skills.
  • Supervisory experience a plus.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

Frequent standing, stooping, bending, stretching, and squatting for approximately 90% of the work day. Must be able to transport residents via wheelchair or shower chair, pushing up to generally 200 +/- lbs. Must be able to transfer, turn and position a resident generally weighing up to 125 lbs. Must be able to lift up to 35 lbs. When lifting over 35 lbs, you are required to ask for assistance. Occasionally push and/or pull wheeled carts and equipment generally weighing up to 200 lbs. May be exposed to communicable diseases including HIV and Hepatitis B viruses; industrial and household chemicals; offensive odors; and combative behavior.
