Reports to: GPD Program Manager I

FLSA Status: Full-time, Non-Exempt

The Lead Case Manager provides comprehensive, housing-focused case management for participants in transitional housing programs, including full assessments of physical, mental, and social needs. This role ensures that participants receive the appropriate services aligned with their individualized service plans and that all actions remain compliant with Grants Per Diem (GPD) Program guidelines. The Lead Case Manager also provides leadership to program staff, supports quality assurance, coordinates services, and ensures participant progress toward housing stability.


Duties and Responsibilities

Case Management & Participant Support

  • Conduct initial and ongoing assessments of participant needs, including housing, income, benefits eligibility, education, life skills, employment, and wellness.
  • Develop individualized housing stability plans using a progressive assistance approach, leveraging each participant’s strengths, and identifying barriers.
  • Conduct regular meetings with participants, review case files weekly, and monitor progress toward goals.
  • Provide crisis intervention and immediate support when necessary.
  • Explain GPD, VA, and community services to participants and establish realistic milestones for stability.
  • Coordinate supportive services within the agency and link participants to public benefits and community resources (e.g., medical, mental health, legal, social services, veterans’ benefits).
  • Maintain accurate, complete, and timely participant records in accordance with agency standards and best practices.
  • Ensure all required documentation is properly maintained in HMIS and Welligent.

Program Leadership & Team Support

  • Work with the Program Manager to hire, train, manage, and evaluate case management staff.
  • Oversee program and contractual outcomes, including planning, workload distribution, and meeting performance benchmarks.
  • Ensure program quality assurance through data tracking, reporting, and file audits.
  • Provide daily support, guidance, and mentorship to case managers, promoting collaboration and a positive team dynamic.
  • Develop and facilitate program trainings and workshops for both clients and staff.

Administrative & Agency Responsibilities

  • Track and record outcome data for required reports.
  • Attend scheduled staff meetings, trainings, and agency-wide events.
  • Share program updates, policy changes, and new procedures with the team.
  • Perform other duties as assigned by the supervisor
Qualifications

Qualifications

Education & Experience

  • Bachelor’s degree in human services, Social Work, or related field preferred.
  • Minimum 2 years of paid case management experience in Human Services, Social Work, or related fields preferred.
  • Minimum 1 year of paid experience in a Housing First program required.
  • Experience with homeless populations is strongly preferred.
  • Basic understanding of the military and the Veterans Administration (VA) required.
  • Veteran status is a plus.

Skills & Competencies

  • Strong written and verbal communication skills; ability to prepare accurate reports and documentation.
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint).
  • Knowledge of HMIS database and Housing First principles preferred.
  • Ability to relate effectively to individuals making major life changes and support their increasing independence.
  • High level of professionalism and ability to maintain strict confidentiality.
  • Strong organizational skills with the ability to prioritize competing responsibilities.
  • Flexibility and ability to adapt quickly to changing priorities.
  • Ability to work both independently and as part of a multidisciplinary team.

Other Requirements

  • Valid driver’s license in good standing.
  • Ability to pass a background check and drug screening.
  • Must be able to fluently read, write, and communicate in English.
  • Must be able to perform all duties both mentally and physically, including sitting for extended periods.

Physical Requirements

  • Ability to bend and lift light objects.
  • Ability to sit for extended periods of time while viewing a computer monitor.
  • Dexterity of hands and fingers to operate a computer keyboard
  • Capable of hearing and speaking to exchange information.
  • Seeing to read reports and data
  • Ability to speak English.
  • Must be able to work in a noisy, busy environment.
