Salary: $28.84 (hourly) Non-Exempt

Other Compensation:

Employer-paid medical for employee and family, employer-paid dental, vision and group life insurance for employee - estimated value $8,243.28 - $23,823.84/annual 

Employer-paid sick pay (12 days per year) - estimated value $2,768.64/annual 

Employer-paid vacation pay (10-20 days per year depending on years of service) - estimated value $2,307.02/annual for first year of service 

Employer-paid anniversary holiday (1-5 days annually depending on years of service) - estimated value $230.72 annual for first year of service 

Employer-paid birthday holiday (1 day annually) - estimated value $230.72/annual

SHIELDS’ mission is to develop, deliver and evaluate culturally sensitive, comprehensive service models that empower and advocate for high-risk families in South Los Angeles.


The Hospital Liaison serves as a key point of contact between the hospital, patients, families, and community resources, supporting program objectives through direct client engagement, coordination of services, and collaboration with healthcare providers. In this role, the Hospital Liaison conducts comprehensive interviews with mothers and families in a nonjudgmental, empathetic, and positive manner while promoting initiatives such as exclusive breastfeeding and access to social services. The position requires accurate data collection and documentation using a Windows-based database system, as well as maintaining thorough records of all client interactions and services provided. The Hospital Liaison builds and sustains strong relationships with hospital providers and works collaboratively across departments to support patient care, facilitate hospital visits, and assist with community outreach efforts. Additionally, the role includes helping to develop and maintain a community resource directory, attending staff meetings and trainings, serving as a single point of contact for post-acute providers, and performing other duties as assigned to enhance patient outcomes.

Qualifications

The ideal candidate will possess a Bachelor's degree in public health, social work, nursing, healthcare administration, or a related field, or an equivalent combination of education and experience. Applicants should have at least one to three years of experience in a healthcare, hospital, community outreach, or social services environment, with preferred experience in maternal and child health, breastfeeding support programs, or case management. Experience conducting client interviews, completing assessments, and working collaboratively with multidisciplinary teams is essential. Strong interpersonal, communication, and organizational skills are required, along with the ability to maintain confidentiality and handle sensitive information with professionalism. Candidates should be proficient in Microsoft Office and comfortable using database systems or electronic health records. Knowledge of community resources and healthcare systems is highly desirable, and bilingual skills, as well as certifications in lactation counseling or community health work, are considered a plus.
