Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Obesity impacts more than 40% of the US adult population, and although historically only about 1% of patients received medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth. Form Health provides high-quality expert care and leverages technology to enhance the patient experience. All Form Health patients work closely with their care team, which includes board certified physicians, advanced practice professionals and Registered Dietitians. Through our proprietary mobile app patients engage in regular video visits, as well as text messaging, photo journaling, digital data transmission, and customized educational materials. About the Role:

We are looking for a passionate and collaborative Data Analyst to power data-driven decision-making across product development, clinical outcomes, operations, and business performance. You will help Form Health scale by building actionable dashboards, uncovering insights, supporting experimentation, and partnering closely with product, clinical, operations, and leadership teams. The role will report to our Lead Data Product Manager, but will work closely with various stakeholders in Product, Engineering, Operations, Finance, and other teams as needed. The position is fully remote and can be located anywhere in the United States.

What You Will Do:

  • Build and maintain dashboards and reporting to track KPIs, clinical outcomes, operational metrics, and financial performance, ensuring timely, accurate insights across the company
  • Collaborate with product and engineering to expose key data in the app, support A/B testing, define and monitor feature success metrics, and inform iterative product improvements
  • Support strategic research and opportunity analysis, partnering with clinical, product, and operational leaders to answer high-priority business questions and surface new opportunities for impact
About you
  • 3+ years of experience in analytics, data insights, or product analytics, ideally in a high-growth, data-driven environment
  • 2+ years of experience working with healthcare data (claims, EHR, EMR, or similar)
  • Strong SQL skills and hands-on experience with BI tools; familiarity with Snowflake and Sigma is a plus
  • Able to turn ambiguous business questions into clear, structured analyses and communicate findings to non-technical stakeholders
  • Comfortable partnering with product, engineering, clinical operations, and finance to drive decisions
  • A proactive self-starter who enjoys solving complex problems in a fast-paced startup setting
  • Highly organized, with a knack for bringing clarity and alignment to cross-functional work
  • Passionate about using data and technology to improve healthcare outcomes for patients
