Job Overview

The Community Liaison's role supports care coordination by closely working with the community healthcare delivery system. This involves facilitating seamless communication and collaboration between healthcare providers, social services, and the community to ensure that members receive comprehensive, coordinated care. By actively engaging with these entities, the liaison helps streamline the delivery of healthcare services, social determinants of health, and resources to the members, ultimately contributing to improved health outcomes and overall well-being within the community. This collaborative approach underscores the importance of the liaison's role in promoting effective care coordination and enhancing the overall healthcare experience for the members.

Minimum Requirements

  • A High School Diploma or GED is required.

  • A minimum of 3 years of experience directly working in the health care environment or a community outreach setting is required.

  • Community Health Worker (CHW) certificate.

  • Preferred experience in a managed health care environment, preferably IPA, HMO, or Health Plan.

  • Preferred experience working with a diverse population.

  • Bilingual English/Spanish is strongly preferred.

Compensation

$26.82 - $33.53 hourly
