Summary of Position

The Clinical Pharmacist for Teladoc Health’s Cardiometabolic Program advances value-based outcomes by leading medication optimization for chronic cardiometabolic conditions (e.g., diabetes, hypertension, obesity, heart failure). Leveraging virtual care workflows and integrated data (claims, Rx, device, and lab feeds), the Clinical Pharmacist analyzes medication utilization to curb polypharmacy, improve adherence, and steer members toward cost effective therapies, as well as evidence-based approaches to high-cost agents, resulting in lower total cost of care. The role collaborates across multidisciplinary teams to design and operationalize interventions, drives measurable improvements in program KPIs, and ensures rigorous documentation and reporting to support performance guarantees in value-based contracts.

Essential Duties and Responsibilities:

Medication Data Analysis & Strategy

  • Analyze medication utilization data to identify trends in polypharmacy, adherence, and high-cost drug use.
  • Develop and implement strategies to reduce unnecessary polypharmacy, improve adherence, and optimize cost-effective therapies.
  • Collaborate with care teams to design interventions that lower total cost of care while maintaining or improving outcomes.
  • Monitor and report the impact of medication management strategies on clinical and financial results.
  • Provide actionable insights to support value-based contracts and performance improvement initiatives.

Population Health & Cost Management

  • Participate in initiatives to improve population health metrics related to medication use.
  • Evaluate prescribing patterns and recommend changes to reduce high-cost drug utilization.

Medication Therapy Management (MTM)

  • Conduct comprehensive medication reviews for patients with chronic conditions (e.g., diabetes, hypertension, obesity, heart failure, asthma, depression)
  • Identify and resolve drug therapy problems; optimize regimens; track and report outcomes and safety metrics.

Education & Collaboration

  • Advise providers on best practices, guidelines, and new therapies.
  • Work closely with physicians, nurses, and care coordinators to integrate pharmaceutical expertise into care planning and transitions.

Quality Improvement & Regulatory Compliance

  • Develop and use metrics to track medication-related outcomes, adherence, and safety.
  • Participate in quality improvement projects and ensure regulatory compliance.

Direct Patient Care (if applicable)

  • Provide evidence-based, patient-centered medication recommendations.
  • Support management of medical emergencies and discharge counseling.

Documentation & Reporting

  • Document all interventions, recommendations, and patient education in the electronic health record (EHR).

Required Qualifications 

  • Graduate of an ACPE-accredited College of Pharmacy; PharmD required.
  • Completion of ASHP-accredited PGY-1 Pharmacy Residency (PGY-2 preferred).
  • Active, unrestricted pharmacy license and have the ability to obtain licensure in additional states without restrictions
  • Minimum 3 years of clinical pharmacy experience in ambulatory care, ideally within value-based care (CIN or ACO).
  • Proficient in contemporary clinical pharmacy practice, medication therapy management, and evidence-based medicine.
  • Skilled in medication data analysis, cost-containment strategies, and quality improvement using analytics tools.
  • Strong understanding of polypharmacy risks, adherence strategies, and cost-effective prescribing.
  • Proficient in medical/pharmacy terminology, computer systems, and EHR documentation.
  • Able to work effectively under pressure and collaborate in a virtual, multidisciplinary environment.
Preferred Qualifications
  • Experience in telehealth, ambulatory care, health plan, or managed care pharmacy.
  • Background in value-based care, population health, or quality improvement projects.
  • Completion of PGY-2 Specialty Pharmacy Residency.

Travel percentage reflects an estimate and is subject to change dependent on business needs.

The base salary range for this position is $110,000- $120,000. In addition to a base salary, this position is eligible for a performance bonus and benefits (subject to eligibility requirements) listed here Teladoc Health Benefits 2026. Total compensation is based on several factors including, but not limited to, type of position, location, education level, work experience, and certifications. This information is applicable for all full-time positions.
