The Canvasser plays a vital role in bringing Wider Circle’s mission to life by building trusted relationships that help neighbors connect for better health. In this role, you will reach out to health plan members through calls, community visits, and local events to promote engagement, provide education on health plan benefits, and connect individuals to valuable resources. You’ll serve as a trusted advocate—helping members navigate healthcare systems, access care, and participate in programs that improve their well-being. This position combines outreach, mentorship, and community partnership to make a lasting difference in the lives of members while strengthening the health of local communities.

Responsibilities

  • Conduct regular outreach calls and in-person check-ins to build rapport, assess member needs, and promote program participation.
  • Recruit and mentor member ambassadors to support outreach and engagement efforts.
  • Develop and maintain partnerships with local community-based organizations to bring additional resources and services to members.
  • Track member engagement, attendance, and satisfaction in the case management system, maintaining accurate and up-to-date records.
  • Achieve membership growth and retention goals while delivering an exceptional experience that reflects Wider Circle’s mission and values.
  • Conduct one-on-one home or hospital visits to educate members on their health plan benefits, available community resources, and strategies to improve their health and well-being.
  • Support members in scheduling primary care visits, completing wellness assessments, and establishing care with their medical providers and care managers.
  • Serve as an advocate by helping members navigate healthcare systems, understand their care plans, and access the support they need.
  • Encourage participation in Enhanced Care Management services and other social activities that promote better health outcomes.
  • Model positive behaviors and provide guidance to help members engage more fully in their care and communities.

Requirements
  • Bilingual (English/Spanish) preferred with excellent communication skills.
  • Community Health Worker (CHW) experience or certification preferred.
  • Strong empathy, compassion, and ability to connect with others.
  • Experience working with Social Determinants of Health (SDOH) and low-income communities is preferred.
  • Previous canvassing, sales, or community organizing experience preferred.
  • Comfortable working independently, remotely, and in the field.
  • Ability to engage one-on-one, build rapport, and motivate others.
  • Familiarity with applications and software; ability to work with remote technology.
  • Reliable transportation to travel extensively to members’ homes and community events.
  • Flexibility to work after-hours, weekends, and outdoors.
  • Ability to stand and walk for long periods.
  • Safely and consistently drive to public places up to 100 miles.

Physical Requirements:

    • Ability to stand, walk, and speak publicly for extended periods.
    • Comfortable carrying supplies up to 30 pounds.
    • Ability to work outdoors and drive to various locations.
