Build community relationships and generate qualified referrals to drive enrollment in WelbeHealth’s PACE program. Coordinate outreach activities, collaborate with senior centers and community organizations, develop monthly marketing plans, achieve enrollment goals, and educate referral sources about services. The role emphasizes relationship-based community engagement rather than hard selling.

The Project Manager leads cross-functional projects and operational improvement initiatives across the organization. Responsibilities include coordinating resources, tracking project status, managing risks, reporting to stakeholders and executives, and ensuring successful implementation. The role also supports PMO infrastructure, improves processes, and collaborates with clinical, operational, and corporate teams in a regulated healthcare environment.

Coordinates participant healthcare appointments, maintains team member availability, handles cancellations and rescheduling, responds to scheduling inquiries, communicates confirmations and reminders, and resolves appointment conflicts with staff, families, and providers. Requires healthcare and EMR experience, strong organization, and excellent customer service and communication skills.
