Develops and runs personalized social and therapeutic activity programs for participants, conducts engagement assessments, participates in interdisciplinary care planning, schedules and supervises group and individual activities, and cultivates community partnerships to support programming.

Provide front-line operational and administrative support for a day center: greet and check in participants, manage scheduling and calendars, handle inventory and supplies, coordinate meetings and events, take meeting minutes, support projects and communications, act as liaison across teams, assist with customer service and service recovery, and manage front-desk security and screening.

Conduct community-based outreach to build relationships with seniors, caregivers, and partner organizations; generate qualified referrals and guide prospective participants into the PACE program; develop monthly marketing plans and meet enrollment targets; provide education about WelbeHealth services.
