Provides nursing coordination, consultation, assessments, and care planning for individuals with intellectual and developmental disabilities. Coordinates with families, providers, hospitals, facilities, and internal committees; evaluates care needs, monitors quality and compliance, supports placement decisions, conducts death reviews, and delivers health education. The community-based role includes home visits, meetings, report writing, and acting as a designee for medical and dental consents.
