Health Insurance + Wellness

Dental insurance

UMR Dental plan to all eligible employees. Premiums for employee only coverage are $6.30 per pay period. Premiums for family coverage are $14.75 per pay period.

Disability insurance

Employees can increase their benefit eligibility amount for STD to 60% of salary or wages up to a maximum of $1,500 per week after a 14-day waiting period. STD coverage covers a 24-week period.

Flexible Spending Account (FSA)

Employees can contribute pre-tax dollars up to a maximum of $2,500 for medical expenses and $5,000 for dependent care expenses.

Traditional PPO and High Deductible PPO. High Deductible PPO plan allows employees to open up a HSA - contribute $3,400 (ee only) or $6,750 (family).

Every employee has a Guardian Basic Life/AD&D Plan in the amount of 150% of annual base salary. Employer covers 100% of premium.

Vision insurance
